- Have opportunities to provide feedback about Vision Loss Rehabilitation Canada and our services by using our online contact form
- Receive any information you request in the communication method you prefer
- Have your personal information kept confidential and secure
We also ask you to adhere to the following important responsibilities and requirements:
- Use our contact form as soon as possible if you need to cancel or reschedule an appointment; 24 hours’ notice is required
- Prepare for and participate in your therapeutic appointments, and reinforce skills through ongoing practice
- Inform Vision Loss Rehabilitation Canada if your personal information changes
- Provide a safe working environment for Vision Loss Rehabilitation Canada specialists when visiting your home
